| CONSENT: I/We (ie: Principal Applicant/Co-Applicant) hereby fully consent to permit the landlords/owners and their designated agents to obtain my/our credit and references information, occupancy history, employment history, and criminal history, if any (including confirming any and all information on this Rental Application Form) for the purpose of determining whether or not to enter into a lease agreement with me/us. Furthermore, I/we fully agree that if accepted, the landlords/owners shall have a continuing right to review my/our on this Rental Application Form and the Rental Agreement, as well as my/our occupancy, employment and criminal (if any) background histories. Furthermore, I/we fully understand and agree that providing false, fraudulent or misleading information, or if any of the information becomes false during the lease term, is grounds for denial of lease or termination of my/our right of occupancy. Furthermore, I/we fully understand and agree that neither John J. Williams nor Laurencia Williams, landlords/owners, nor their designated agents, heirs or assigns shall be liable to anyone for any claim, injury or damage consequent upon furnishing the information requested in this Rental Application Form or acting on it. Furthermore, I/we fully understand and agree that , if accepted, and if the landlords/owners permit me/us to keep an animal(s) or to operate a business on or from this property that their permission does not constitute any form of government permission or approval. Furthermore, I/we shall fully indemnify, hold free and harmless and defend the landlords/owners from any and all liabilities, judgments, damages, injuries, losses and costs that may result directly or indirectly from the animal(s) I/we permit on the property, from the business activities I/we conduct or permit from or on the property, from all losses and damages done to the property that exceed both normal wear and tear and my/our deposit, and from all intentional and negligent acts I/we do on the property. Furthermore, I/we assume full responsibility for all intentional and negligent acts committed on the property by ourselves and by all others (both human and animal) we permit to live or visit on the property. Furthermore, I /we fully understand and agree that nothing stated or possibly implied herein shall constitute an offer of employment, partnership, joint venture or other business relationship between me/us and the landlords/owners except that which normally exists between landlord/owner and tenant. |
| ACKNOWLEDGMENT and QUALIFICATIONS: The landlords/owners do not have a duty to verify, and they neither represent or covenants that they will verify, the accuracy of the answers provided to the foregoing questions concerning prospective residents. Furthermore, the landlords/owners have no duty and expressly disclaim any obligation to perform a criminal, credit or any other background check on Principal Applicant and/or Co-Applicant for lease or continued occupancy purposes. The ability of the landlords/owners to run criminal background checks does not constitute any guarantee, assurance or representation that the Principal Applicant or Co-Applicant have no prior criminal record or background or that the landlords/owners have undertaken any effort verify the lack of criminal record or background of the Principal Applicant or Co-Applicant. If accepted, any inadequacy, omission or error that the landlords/owners may have in verifying any of the information provided in this Rental Application Form or in acting on or enforcing any provision of this Rental Application Form does not waive, diminish or forego any rights of the landlords/owners, including their right to verify this type of information or to act on or enforce any provision of this Rental Application Form and Rental Agreement Form in the future. By signing below, Principal Applicant and Co-Applicant represent that all of the data provided in this Rental Application Form is true and correct, the Principal Applicant and Co-Applicant understand that the landlords/owners heavily rely on this data and its accuracy and completeness as attested to by the Principal Applicant and Co-Applicant The landlords/owners expressly reserve the right to cancel any lease if Principal Applicant or Co-Applicant has made any false or misleading statements in this Rental Application Form without liability to them. |
| MOISTURE AND MOLD: As with any property, there may be moisture and mold issues at the property in the future and your cooperation and coordination with the landlords/owners inspection processes and any necessary and appropriate corrective action may be required. Should you have any maintenance issues regarding moisture and/or mold growth, you must promptly report them to the landlords/owners so that they can be addressed promptly and properly by them. |
| LEAD PAINT ISSUES: By law, if the home is older than 1978, which this home is, the landlord/owner is required to provide the tenant with both a Lead-Based Paint Disclosure and the Government's "Protect Your Family From Lead in Your Home" EPA pamphlet prior to signing the Rental Agreement (this is the Rental Application Form - not the Rental Agreement Form). Landlords/owners have no knowledge of lead-based paint and/or lead-based paint hazards in this housing, and they have no reports or records pertaining to lead-based paint and/or lead-based paint hazards in this housing. The Disclosure and Pamphlet state that lead paint, lead paint chips, and lead paint dust can pose health hazards if not managed properly, and that lead exposure is especially harmful to young children and pregnant women. |
| RENTING OR LEASING TO OTHERS NOT PERMITTED: If accepted, I/we fully agree that I/we shall neither rent nor lease all or any part of this real estate property to anyone. This does not mean that other family members cannot live with you; if any do, you must accurately disclose all to the landlords/owners in writing within 30 days, you cannot charge them rent to live with you, you must live in the home during their entire period, and the total number of people living in the home cannot exceed the maximum occupancy at any time. |

By signing this Form, each Applicant attests that he/she has read, understood and accepted without qualification all 3 pages of this 3-page Form, and he/she gives landlords/owners full permission to obtain his/her/their credit report and all other background histories as described herein. Along with this properly signed and dated Form (include all additional attachments you need to include with this Form), each Applicant must also provide a clear unredacted photocopy of his/her current driver's license (if Applicant does not have driver's license, clear unredacted copy of other official picture ID will do). Landlords/owners confirm that the above is the genuine written permission of the above applicants. Landlords/owners understand it is a violation of the Federal Fair Credit Reporting Act to obtain a credit report on any person without their written permission or legitimate business purpose, which landlords/owners can document with this signed and dated form. Landlords/owners can obtain credit reports on anyone, but only with their written permission or other proof that landlords/owners are in compliance with the Fair Credit Reporting Act. If landlords/owners turn down an applicant based on his or her credit, he or she is entitled to know why and get the name of the credit reporting agency.
